Furthermore, most first aid kits are not supplied with proper tools for managing dental problems unless a traveler has had the foresight to carry the necessary supplies and learn how to use them properly.One traditional concept in dentistry has been that certain antibiotic coverage is necessary for persons with certain structural heart problems prior to certain dental manipulations. I was taught this in medical school, and dogma about this has notchanged until just recently. The American Heart Association (AHA) has released new recommendations that state that the use of antibiotics prior to dental procedures is rarely needed. This information is applicable to the wilderness as well as the urban setting.The update states that giving antibiotics to patients prior to dental procedures is unlikely to prevent many cases of infective endocarditis [inflammation of the lining of the heart and its valves].A specially appointed writing group sponsored by the AHA was created because of its collective expertise in prevention and treatment of infective endocarditis. This group, along with members representing the American Dental Association, the Infectious Diseases Society of America, and the American Academy of Pediatrics, concluded that the use of prophylactic (preventive) antibiotics should be restricted to patients at risk for infection, such as those with artificial heart valves or certain congenital heart defects.

Today a 31 year old female presented for an external bleaching. The case was interesting in that a maxillary central incisor presented with much darker staining due to a previous root canal therapy treatment completed at a different office. The goal of the external bleaching performed today was to not only to lighten all of the teeth, but to attempt to gain a few shades of whitening for this darkened tooth in particular. The teeth were properly isolated with liquid dam material. At the beginning of each cycle of bleaching, the darker central incisor was bleached for 4 minutes. Then bleach was added to the remaining teeth and the teeth were bleached for 12 minutes. Two cycles were completed. Upon completion the central incisor had lightened several shades. The patient had little post-op sensitivity. She was happy with the esthetic results

No specific treatment of mandibular dislocation is indicated in the field. The decision regarding self-transport versus paramedic transport is based upon factors other than the mandibular dislocation (eg, presence of multiple trauma, patient's level of pain and distress).

Emergency Department Care

A thorough assessment of the patient’s airway, breathing, and circulation (ABCs) should be performed at presentation. If a complete history, physical assessment, and appropriate imaging study reveal an isolated mandibular dislocation, a decision is to be made if closed reduction in the emergency department is appropriate. Oral maxillofacial surgery consultation is indicated for patients with dislocations associated with fractures and for chronic dislocations. Based on the degree and displacement of the fracture and damage to associated structures, many of these patients require open reduction in the operating room.
Providing analgesia and muscle relaxation prior to reduction is important. Several options are available including procedural sedation using a combination of intravenous sedatives and analgesics. Local anesthetics (eg, lidocaine) can be injected directly in the TMJ space at the site of the preauricular depression.17 A short-acting benzodiazepine, such as intravenous midazolam, can be used for muscle relaxation. Several methods have been proposed and successfully used for reduction of anterior jaw displacement.

Wisdom teeth are the third and final set of molars that most people get in their late teens or early twenties. Sometimes these teeth can be a valuable asset to the mouth when healthy and properly aligned, but more often, they are misaligned and require removal.
Wisdom teeth present potential problems when they are misaligned – they can position themselves horizontally, be angled toward or away from the second molars or be angled inward or outward. Poor alignment of wisdom teeth can crowd or damage adjacent teeth, the jawbone, or nerves. Wisdom teeth that lean toward the second molars make those teeth more vulnerable to decay by entrapping plaque and debris. In addition, wisdom teeth can be entrapped completely within the soft tissue and/or the jawbone or only partially break through or erupt through the gum. Teeth that remain partially or completely entrapped within the soft tissue and /or the jawbone are termed "impacted." Wisdom teeth that only partially erupt allows for an opening for bacteria to enter around the tooth and cause an infection, which results in pain, swelling, jaw stiffness, and general illness. Partially erupted teeth are also more prone to tooth decay and gum disease because their hard-to-reach location and awkward positioning makes brushing and flossing difficult.

How Do I Know if I Have Wisdom Teeth?

Wisdom teeth present potential problems when they are misaligned – they can position themselves horizontally, be angled toward or away from the second molars or be angled inward or outward. Ask your dentist about the positioning of your wisdom teeth. He or she may take an X-ray periodically to evaluate for the presence and alignment of your wisdom teeth. Your dentist may also decide to send you to an oral surgeon for further evaluation.
Your dentist or oral surgeon may recommend that your wisdom teeth be extracted even before problems develop. This is done to avoid a more painful or more complicated extraction that might have to be done a few years later. Removal is easier in young people, when the wisdom teeth roots are not yet fully developed and the bone is less dense. In older people, recovery and healing time tend to be longer.

Dental Pulp is the soft tissue that contains nerves, blood vessels and connective tissue. It lies within the tooth and extends from the crown of the tooth to the tip of the root in the jawbones.

How is the Pulp Damaged? When the pulp is diseased or injured and can't repair itself, it dies. The most common cause of nerve death is a cracked tooth or a deep cavity. Both of these problems can allow germs (bacteria) to enter the nerve.

These can be found in any local drug store. If the sensitivity is uncomfortable, use an over-the-counter pain medicine, as directed

For example, carrying this animal tooth shown in the foreground on the right was believed to cure toothache. It was hoped that the pain would be transferred from the person to the tooth. It wasn’t always an animal tooth that was used; it was not unknown for a human tooth to be taken out of a skull from the local churchyard to perform the same function. The stone was a gift to the Wellcome collections in 1916 from Edward Lovett (1852-1933), a collector of British amulets and charms. it is pictured here with four other amulets against toothache: a large animal tooth (A132541), two stone amulets, (A132503 and A132474) and a triple hazelnut (A132536).
